Recognition of the impact of social determinants of health (SDOH) on patient outcomes is growing, as is the desire to incorporate SDOH factors (e.g., neighborhood and housing, income, education, food security, social support, transportation, and access to technology) into care plans.
Terminology to document and communicate these factors plays an essential role in evaluating and addressing the significant impact of these socioeconomic and environmental factors on patient health. To be accounted for in clinical settings and transformational to whole-person clinical care, SDOH information needs to be coded and interoperable across the health and human services ecosystem using standard coded terminologies and standards for exchange.
As the steward of the Current Procedural Terminology (CPT®) code set and a member of the Gravity Project (an SDOH collaborative), the American Medical Association (AMA) is a driver in the effort to create and standardize the coding for social risks and needs.

Corey Smith Vice President, Informatics & Digital Products American Medical Association Corey Smith is the vice president of Informatics and Digital Products for the American Medical Association’s (AMA) Integrated Health Model Initiative (IHMI). In this role, he and his team focus on improving patient health outcomes by empowering physicians with clinically valid health care data and software solutions needed to make informed clinical decisions. The IHMI Team approaches this work by developing and validating data portability standards including HL7 FHIR and SNOMED CT and through the creation of value-added software solutions.
Prior to joining the AMA over seven years ago, Corey led product management for Health Language, Inc., a healthcare data quality solutions vendor, now owned by Wolters Kluwer. He started his career in informatics more than 20 years ago as a software and data engineer for SNOMED International, then a division of the College of American Pathologists.
He has served as a member of the SNOMED CT Design Team, chaired the SNOMED CT Release Format V2 working group, and was an appointed member of the SNOMED CT Technical Release Advisory Group. He has worked on standardized terminology development and implementation projects around the world for national governments, payers, providers, and software vendors.
Corey holds a BA in Psychology from Northwestern University and MS in Computer Science from University of Chicago.
Sarah C. DeSilvey, DNP, FNP Director of Clinical Informatics The Gravity Project Sarah C. DeSilvey, DNP, FNP, balances practice as a rural family nurse practitioner with advocacy for addressing the social determinants of health in clinical and community practice. Her area of focus is developing the terminology to capture the process of caring for patients’ social needs. Her work has now evolved into The Gravity Project—a national consensus initiative to develop terminology to address social needs. She serves as The Gravity Project’s director of clinical informatics, helping to weave the practice of social care, community-based approaches, and the literature of social risk into health care terminology.
Leslie Prellwitz, MBA, CCS, CCS-P Director, CPT® Content Management & Development American Medical Association Leslie Prellwitz, MBA, CCS, CCS-P is the director of CPT Content Management & Development at the AMA. Her work focuses on setting the strategic direction for publishing and maintenance of CPT content, as well as serving as secretary to the CPT Assistant Editorial Board; and providing advice on the proper use and application of CPT to key stakeholders, including educational content presented in CPT Assistant and Clinical Examples in Radiology. Certified in both CPT and ICD-10 coding, Leslie has over 25 years of experience in healthcare management, coding analytics and consulting, with a focus on the academic medical center environment.
