Hillary Lum, MD, PhD is an internist, geriatrician and palliative care physician at the University of Colorado School of Medicine, University of Colorado Hospital and Rocky Mountain Regional VA Medical Center at the Eastern Colorado Geriatrics Research Education and Clinical Center. Since moving to Colorado in 2011 from the upper Midwest, she enjoys being a primary care geriatrician, directing a telehealth program for rural older Veterans, and conducting research to improve care for older adults with serious illnesses. She designed and implemented a novel advance care planning group visit intervention for primary care. In 2017, her team led the implementation of UCHealth MyHealthConnection advance care planning tools. She is passionate about working with community partners including Colorado Veteran Community Partnership, CIVHC’s Advance Care Planning Stakeholder Workgroup and serving as a Colorado representative to the National POLST Assembly. She and her husband enjoy living in Aurora with their 2 dogs and exploring the many ethnic dining options.
