The Use of a Safety Management System in the Oil & Gas Industry and Beyond

Abstract:

In October of 2019, an employee and their spouse were fatally injured after being exposed to high levels of hydrogen sulfide (H2S). This tragic, yet fully preventable, incident did not have to happen. Using the CSB released final report on the Aghorn Operating waterflood station hydrogen sulfide (H2S) incident, this webinar will review the findings of the investigation, probable causes, and key learnings, then discuss how they can be applied to the O&G industry, as well as other industries, that have H2S and non-H2S operations. We will review information from the CSB Staff presentations and the final report issued May 2021.

Wayne Vanderhoof is a Certified Safety Professional with more than 30 years of experience in public safety, mechanical engineering design, and worker safety. For the past 13 years, Wayne has been the Senior Consultant and President of RJR Safety Inc. a safety consulting practice in the manufacturing, oil & gas, and construction industries advising company executives, management, and workers on issues relating to occupational safety and health including OSHA compliance, injury prevention, and loss control and prevention. Mr. Vanderhoof has worked extensively with clients to ensure compliance with safety requirements and OSHA regulations, conduct safety audits, develop safety programs and procedures, and implement safety processes as required. His expertise includes conducting compliance inspections; creating and delivering a variety of safety training programs; and investigating incidents, analyzing root causes, and developing corrective action plans.

Mr. Vanderhoof has a Bachelor of Science degree in Occupational Safety and Health from Columbia Southern University, is a Professional Member of the American Society of Safety Professionals (ASSP), a Co-Chair of the Safety Committee for Pennsylvania Independent Oil & Gas Association (PIOGA). Wayne is an adjunct Instructor for West Virginia University, an adjunct Professor at Thiel College, an Authorized OSHA Outreach Instructor, authorized SafeLand USA Instructor, and Train the Trainer Instructor for Hydrogen Sulfide Safety Training through West Virginia University.

Wayne is a member Standards Committee for ANSI/ASSP Z390.1 – Accepted Practices for Hydrogen Sulfide (H2S) Training Programs, a member of the Standards Committee for ANSI/ASSP Z490 - Criteria for Accepted Practices in Safety, Health and Environmental Training, and a past member of the development group for American Petroleum Institute (API) Recommended Practice 99 - Flash Fire Risk Assessment for the Upstream Oil and Gas Industry.
